血圧とは？
血圧とは、心臓から血液が全身へ行き渡るときに掛かる血管への圧力のことです。
一般的に2つの血圧があり、収縮期血圧（上の血圧とも呼ばれる）は、心臓が収縮をして血液が勢いよく行き渡るため、血管への圧力がもっとも高くなるタイミングの血圧です。
拡張期血圧（下の血圧とも呼ばれる）は、心臓が拡張をして全身から戻ってくる血液をため込むときに最小の圧力が掛かるタイミングの血圧を指します。
現在、日本の高血圧者数は推定4,300万人※1） と言われており、日本国民全体のおよそ3人に1人が高血圧者と推定されています。

血圧の基準値は以下の表をご参照ください。

日本高血圧学会のガイドライン※2） では、収縮期血圧が140 mmHg以上、または拡張期血圧が90 mmHg以上の場合を高血圧と診断します。
自宅の場合は家庭血圧と呼ばれ、診察室血圧よりも5 mmHg低い基準値で診断されることが特徴です。

ランニングのとき血圧は上がる？下がる？
ランニングのときの血圧の変化を簡単に説明すると、ランニング中は血圧が上がり、ランニング後は血圧が下がることが一般的です。
ではなぜ、このような血圧の変化が生じるのでしょうか？
以下でそれぞれのメカニズムをご紹介します。
ランニング中に血圧が上がるメカニズム
ランニング中は一般的に心拍数が上がります。心拍数が上がると心臓から多くの血液が全身へ送りこまれ、血管内への圧力が高まるため「ランニング中の血圧は上がる」というメカニズムです。
また、ランニング中は自律神経のなかの交感神経が優位になっているため、体が戦闘モードに入り血圧や心拍数を上げる特徴があります。
これは、ランニングにかぎった話ではなく、運動全般におけるメカニズムです。
ランニング後に血圧が下がるメカニズム
ランニング後は心拍数を下げ、筋肉に酸素や栄養を補給しなければならないため血管が広がり血圧が下がります。
また、ランニング中に働いていた交感神経が落ち着き、体をリラックスさせようとする働きも、ランニング後に血圧が下がる理由のひとつです。

高血圧者・低血圧者はランニングをしたほうがよい？
結論から申し上げますと、高血圧者がランニングをすることで血圧を下げる効果が期待できます。つまり、高血圧者でもランニングはしたほうがよいでしょう。
ただしランニングをおすすめするのは、先述した高血圧の診断基準のⅡ度高血圧（脳心血管系疾患のない人が対象）まで。Ⅲ度高血圧以上の高血圧者はランニングをはじめとした運動療法よりも降圧薬を用いた治療を優先して行うことが高血圧ガイドラインで定められています。

高血圧者におすすめのランニング強度と頻度
厚生労働省提供のe-ヘルスネット※3）では、高血圧症を改善するための運動として
- ウォーキング（速歩）
- ステップ運動
- スロージョギング
- ランニング
などの有酸素運動を推奨しています。
ただし、高強度のランニングは血圧が上がりすぎるため中強度のランニングが推奨されています。
中強度のランニングとは最大酸素摂取量（Vo2max）の40～60％。感覚でいえば、軽く息があがりはするも、隣の人と会話ができるほどの強度です。
福岡大学の進藤宗洋教授、田中宏暁教授らが提唱する「ニコニコペース」というものを意識するといいですね。
ニコニコペースの求め方は、「138−（年齢÷2）」が運動中に目標とする脈拍になります。
ランニング中に心拍係数を腕に付けて測りながら走るのもひとつの手です。
ランニングの頻度は30～40分ほどをできるかぎり毎日。30～40分を続けて実施できない場合は、10分以上のランニングを分けて実施しても高血圧の改善には効果的とされています。

低血圧者におすすめのランニング強度と頻度
生活習慣病とも呼ばれ、日常生活の問題で発症する高血圧と違い、低血圧は遺伝による発症が多いとされています。
そのため、ランニングをはじめとした運動療法の効果に着目した論文や報告が少ないことが事実ですが、一般的には低血圧者においてもランニングは有効だと判断されています。
ただし、めまいや立ち眩みを感じたときは、ただちにランニングを中止すること。また、ランニング強度もはじめは低～中程度とやや抑え気味に実施して、慣れてきたら徐々に強度を上げていくとよいでしょう。
スポーツ心臓と血圧の関係性
水泳・マラソン選手に多いといわれるスポーツ心臓。
長年のトレーニングを積み重ねた結果、心臓が普通の人よりも大きく肥大している状態を指します。
心臓が大きい分、1回の拍動でより多くの血液を全身へ送りこむことができるため、拍出量が少なくても済むため、心拍数が40～50回/分と少ないことが特徴です。
スポーツ心臓になると、血圧は少し上がるか通常と変わらないといわれており、人によって異なることがわかります。

正しい血圧の計り方
正しく血圧を計ることで、より正確な血圧を把握できます。
ここでは、高血圧治療ガイドラインをもとに、正しい血圧の計り方をわかりすくお伝えさせていただきますので、ぜひご自宅でも実践してみてください。
【測定位置】
上腕部。
※心臓の高さに近い位置が望ましいです。
【測定時の条件】
朝：起きて1時間以内、排尿後、服薬前、座位で1～2分間安静にした後に測定。
晩：就寝前、座位で1～2分間安静にした後に測定。
血圧測定はできるかぎり同じ条件のもと毎日測定することが望ましいです。
また、家庭用の血圧計はさまざまな種類がありますが、できるかぎり腕に巻くタイプの血圧計を選ぶようにしましょう。
